All adult volunteers at St. Jude who work with children must fill out an EAPPS Application, sign the Code of Ethics, and complete Safe in Place online training.

EAPPS Application Instructions:

a)  fill out all items completely (can skip Employment history, Educational history and Volunteer history)

b)  if your current residence is less than 5 years, you must include your previous address

c)  for the "References" section, all addresses MUST be complete including street, city, state & zip and phone number

d)  for the “Background check” section, we MUST have either your social security number or Driver’s License number

e)  Please sign page 18 for the Code of Ethics Acknowledgement

f)  application is NOT complete until Safe in Place online training is completed (instructions on last page of packet) - please do not turn in application until you have a copy of the training certificate or it has been emailed to me at christieg@stjudecatholic.org.

g)  return completed forms and certificate (if not emailed) to:  St. Jude Church, Attn: Christie Guillory, 9150 Highland Road, Baton Rouge, LA 70810
